From social care to social housing.
Springplank040 is responsible for the social care in Eindhoven.

| Focus on providing livelihood security for all

For us, the focus within social care is on providing livelihood security for all. The aim is to create a route to livelihood security, focusing on the three essential living areas in the Springplank needs pyramid: living, working and participating in society.

Springplank040 moves away from fixed pathways. We focus on individual pathways, with counsellors providing tailor-made solutions and the person in question determining his or her own goals and capabilities. Our vision focuses on creating opportunities and perspective for people without housing and employment, with trust as the basis for this collaboration.

| Customised route at springplank
While people stay in the 24-hour shelter, they receive an intake at the Intake and Advice Team. They work with them to see which tailor-made route is the best fit. We aim to move on as quickly as possible.

Finally, there is a recommendation and the candidate is paired with a route counsellor. The route counsellor gives structure to the candidate so that he/she can start working on the future. We guide the candidate towards participation in various forms and search for housing. In the tailor-made route, a counsellor looks at the following living areas with the candidate.
| Sustainable outflow
On average, a tailor-made route lasts between six months and two years, after which the candidate moves out to their own home, room or other suitable housing.
